The Online Application Form is in 4 sections. Please read through the form initially and prepare your text for entry under the various captions. It may be helpful to have your descriptions prepared in a word processing package so that you can copy and paste, then enter the data and complete the form in one sitting.

Do not use ‘Enter’ on your keyboard except when in boxes of more than one line as this will clear the form. Use the mouse to move between captions.
Click on any boxes that apply and leave blank if not applicable.

We can also E-mail a simple text-based Application Form. This has the advantage in that it can be treated as any word processing document and saved on your computer. The form can then be completed in stages, before being E-mailed back to us as an attachment. Alternatively, we can post this form to you to be completed by hand.
